Child, hospitalized in the intensive care unit of the hospital, «Hadassah Ein Kerem» in Jerusalem and connected to the ventilator, was by mistake connected to the balloon with helium instead of oxygen.
According to «Israel a-Yom», the incident occurred in December 2017 and, fortunately, does not entail any consequences for the health of the child. Nurse office noticed the error the employee who brought the helium tank instead of a mixture of helium and oxygen, and turned off the wrong cylinder.
The Ministry of health was informed about the incident. The audit showed that the cylinders are the same size and marked by a marking of the same color, which led to the error. In addition, before connecting a gas cylinder had not conducted a reconciliation of data.
After the inspection, it was recommended to store cylinders of different gases in different places and to instruct workers concerning reconciliation before connecting the cylinders.
